Hi in researching a patent smiler to the one I would like to file

under Transaction Description

the last office action was

Mail Abandonment for Failure to Respond to Office Action

Does this mean that it was never approved?

Yes but be careful about the date.  Normally they have up to 6 months to respond so you cannot say for 100% that is is abandoned yet.
