You can use the USPTO "public PAIR" system to lookup an already approved Design patent by their patent number.   Once in their system you can see the exact files that were used to file the application and use those as examples:

Hello everyone I am looking for a solid example of a completed design patent application.
No, not a processed patent (with the USPTO barcode across the top), but the application write-up.
I am skilled at completing the necessary pdf forms (SB16 / SB15 etc).
I need an example of an actual document that is turned in with the forms ( the preamble, claims etc;)
There does seem to be a template or example anywhere in pdf or word format.
